Surgical Necklift

Multiple factors play a role in how we look as we age. The subcutaneous tissue in the neck decreases in volume causing the skin to look wrinkled and droopy. Besides volume loss, excess sun exposure will cause a breakdown of elastin and collagen in the skin, making it sagging even more. Surgical necklift will help to address this skin sagginess by removing the excess skin and re-suspending the neck muscles that have fallen. The anterior and posterior necklift approaches are alternative operations for patients who request more conservative procedures with faster recovery than traditional facelift surgery.

Anterior Necklift (Submentoplasty)

A submentoplasty is an abbreviated type of Neck lift that is mostly used on young patients. It is an effective procedure with isolated submental fullness without significant jowling or midface aging changes. It involves surgery of the area under the chin and the upper portion of the neck. Submentoplasty should be used in patients who need more than a simple liposuction but less than a facelift. Liposuction is also performed along the inferior border of the mandible and in the jowl region if necessary. The best candidate is a younger patient with fat deposits in the neck but without a lot of skin excess.

 The procedure involves making a small incision under the chin, liposuction is performed where needed, then the platysma muscles are reduced, fat underneath the muscles is also remove, then the muscle edges are sutured back, tightening the neck. This is why submentoplasty can be performed without cutting away skin. This mini neck lift procedure takes about one hour. Posterior Necklift

Patients’ concerns regarding the appearance of their saggy neck often stem from increasing skin and muscular laxity. This common patient concern continues to increase due to obesity, ultraviolet radiation and aging population. Although various forms of noninvasive and minimally invasive techniques are utilized to enhance the appearance of the neck, the surgical neck lift is the most efficacious procedure to reduce the appearance of significant sagging jowls and skin laxity. This necklift technique is designed as less invasive procedure compared with the traditional facelift and has limitations. As described, it addresses neck skin and some degree of jowling. Since there is no preauricular incision, the cheeks and jowls are less influenced. Concomitant submental liposuction and midline platysmaplasty can be performed in patients with platysmal banding and increase submental fat deposition.
