Scar treatment for acne and traumatic or surgical scars is a common request in cosmetic practice. The CO2 laser and fractional radiofrequency resurfacing are predictable and effective treatment for various types of scar.
Patients who suffer from acne have unsightly scars and bumps that not only affect the appearance of their skin, but also causes psychological issues. Before the advent of lasers and radiofrequency therapy, deep chemical peeling and dermabrasion were the mainstay therapies. Radiofrequency and CO2 lasers have been described as the most effective tools in acne treatment, over the past decade. Acne scar treatment is rarely a single procedure and frequently requires numerous treatments, depending upon their severity and acne scar configuration.
The CO2 laser approach, remains the gold standard of treatment. Most significant or deep scars do not respond well to superficial fractional laser treatment, the aggressive CO2 laser approach has better efficacy. The goal is to level the skin surface to make the depressions less noticeable. Before treating acne scar, any active acne must be under control. Besides, patients can not be on systemic retinoids (Accutane) for at least one year.
The possible complications associated to skin resurfacing are herpetic infection, bacterial infection, milia formation, hyperpigmentation, hypopigmentation, acne flair burns, scars and tissue adhesion in the healing lower eyelid skin. To prevent hyperpigmentation, we recommend to pre-treat the skin with hydroquinone, vitamin C, kojic acid, or tretinoin/retinol based products three weeks before treatment.
Radiofrequency with microneedling not only creates microscopic columns piercing the skin that resemble a fractional laser pattern when the skin is treated but also produces a volumetric tissue heating into the reticular dermis which causes tissue tightening, increased collagen production and skin remodeling. The advantage of Fractional Radiofrequency ablation over fractional laser is the minimal post-operative redness and the ability to be used in darker skin patients.
These two technologies are available at Slim Image Aesthetic Center. Treatment is customized to optimally meet the needs of each patient by tailoring treatment efficacy and safety. In general, Laser treatment can be used in skin types I to III. Radiofrequency (RF) is safer for patient with skin type IV to VI. Patients will need multiple treatments based on the severity of their acne.
